Thanks for the reference. I have also found USNO circular 179 from  
George Kaplan quite useful on this topic (see  
<http://aa.usno.navy.mil/publications/docs/Circular_179.php>, mainly  
section 1.2).

I have fixed the issue and in fact it allowed me to discover several  
closely related issues at the same time. I'll record the issues to the  
forge and I'll commit the fix in the next few minutes.
